In order to draw up the social documents, please send us:
- Your postal address | - Your telephone number | - A copy of your SS card | - A RIB for the payment of salaries | - A copy of your identity card
Health insurance
Affiliation to the HCR Santé scheme must be made through your employer. Please also send us a bank details slip and a Social Security certificate

If you have individual cover, you can request your insurer to terminate the contract amicably if you can prove that you are obliged to be affiliated to a group scheme. To do this, you must send your termination request to your insurer, enclosing the certificate issued by your employer. (templates below)
